    Position Overview: 
    The First Connections Community Nurse will be part of a collaborative, multi-disciplinary team – consisting of nurses, social workers, and community health workers – providing short-term home-based services to at-risk families with a focus on children between the ages of 0-3, as well as those involved with child protective services. 
     
    Provides clinical knowledge, expertise in assessment, relationship-building skills, and knowledge of area resources to support, educate and empower families to achieve their full potential.
     
    Provides home-based services, which includes assessing maternal, child, and family needs, facilitating linkages between families and services and coordination of care with internal team members as well as external providers, including but not limited to pediatricians, medical homes, mental health providers, and the Department of Children, Youth and Families.
     
    Participates in community networking activities, task forces, and other community-based opportunities based on program and or agency needs.
     
    Maintains a comprehensive knowledge of –and ensure effective referrals to – community resources, including WIC, long term home visiting, affordable housing and other entitlement and community programs.
     
    Ensures that all families of babies with significant metabolic or hemoglobinopathy test results receive necessary follow-up care.
     
    Develops relationships with Community providers to support the referral of clients as well as facilitate the acceptance and completion of home visits.
     
    Maintains client records and complete all necessary documentation within established program time frames.
     
    Adheres to productivity requirements as established for the First Connection Program.
     
    Constructively utilizes formal and peer supervision to evaluate and improve effectiveness in providing services to clients.  This includes active participation in supervision, team meetings and agency meetings.
     
    Participates fully in Children’s Friend’s Universal Training Curriculum, which provides intentional, practical and reflective training to ensure the agency maximizes the capacity and expertise of each staff member, both individually and as part of the overall Children’s Friend team.   
     
    Develops and maintains ongoing outreach strategies to inform families and the community of agency programs, services, and activities offered through Children’s Friend.
     
    Maintains electronic and hard copy client records, completing all necessary documentation within established program time frames.
     
    Ability to interact with members of the community representing diverse backgrounds and interests.
     
    Miscellaneous duties as assigned.

    Qualifications: 
    • Associates Degree in Nursing required; Bachelor’s Degree preferred.
    • Rhode Island Nursing licensure (RN) or LPN required.
    • Must have demonstrated interest in maternal child health.
    • Home visiting experience in working with vulnerable children and/or families preferred. 
    • Must have strong organizational skills, computer skills, and excellent oral and written communication skills. 
    • Must have the ability to support flexible work hours.
    • Bilingual preferred.
    • Must have a valid driver’s license and automobile liability insurance.
     
    Hybrid Eligible:
    Given the nature of this role, the First Connections Community Nurse will need to be able to work an onsite and remote work schedule.
